Hiring: Caseworker | Case Manager, Community Programs

Job Type: Part-time

Location: Lehigh Valley, Pennsylvania 18102

Abraxas Lehigh Valley Community Programs is hiring a Case Manager to maintain a caseload of youth in need of additional community and in-home support involved in Children & Youth or Juvenile Probation.

This role requires daily travel throughout the Lehigh Valley in both Lehigh and Northampton Counties. The Case Manager will arrange and conduct home visits with youth and/or their families and develop and implement appropriate service plans. You will support clients and their families in completing court-ordered conditions such as assessments, community service, schooling, and counseling. Additionally, you will develop and maintain community resource guides for clients and utilize group curriculum to facilitate group sessions.

As a Case Manager, you will supervise client progress and prepare required reports on a daily, weekly, and monthly basis. Key areas of responsibility include working on truancy prevention, addressing parent-child conflicts, supporting at-risk youth, providing parenting guidance, and handling various case management needs. Strong organizational and communication skills are required.

Case Manager Basic Primary Duties and Responsibilities:

  • Maintain a client caseload with appropriate service plans.
  • Enforce program norms, rules, and behavioral standards.
  • Coordinate and develop appropriate service plans using identified resources for client transitions.
  • Supervise client activities and prepare reports on their behavior and progress.
  • Support clients and families in completing court-ordered conditions.
  • Coordinate with courts, schools, and social service agencies to meet client needs.
  • Maintain regular contact with referral agency staff.
  • Develop and maintain community resources and program curricula.
  • Attend court hearings, school meetings, and home visits.
  • Provide transportation for clients as needed.
  • Develop a weekly work schedule to service clients optimally.
  • Promote the Abraxas philosophy and the Seven Key Principles of care.
  • Demonstrate Safe Crisis Management techniques.

Case Manager Hiring Requirements:

  • Bachelor's Degree in a related field and one year of experience with youth.
  • At least twenty-one (21) years of age.
  • Background checks required.
  • Pre-employment physical exam and drug screen.
  • Valid driver's license and vehicle insurance.
  • Ability to work non-traditional hours to meet client needs.
