*Job Overview*
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will have experience in various settings, including acute care, outpatient clinics, and home community care. This role involves assessing patient needs, developing personalized treatment plans, and providing high-quality rehabilitation services to enhance patient mobility and overall health.

*Duties*
* Conduct comprehensive evaluations of patients' physical abilities and limitations.
* Develop individualized treatment plans based on patient assessments and goals.
* Implement therapeutic interventions such as trigger point therapy and discharge planning.
* Collaborate with other healthcare professionals to ensure coordinated care for patients.
* Educate patients and their families about treatment processes, exercises, and self-care strategies.
* Maintain accurate documentation of patient progress and treatment outcomes in accordance with clinic systems.
* Provide therapy in various settings, including inpatient facilities and outpatient clinics.
* Stay updated on medical terminology and best practices in physical therapy.

*Requirements*
* Valid state license to practice as a Physical Therapist.
* Strong knowledge of acute care practices, outpatient therapy techniques, and home community care protocols.
* Familiarity with medical terminology relevant to physical therapy.
* Excellent communication skills for effective interaction with patients, families, and healthcare teams.
* Ability to work collaboratively within a multidisciplinary team environment.
* Experience with discharge planning processes is preferred but not required.
* Commitment to ongoing professional development and staying current with industry advancements.

Pay: $42.00 - $44.00 per hour
