Qualifications:
- Valid LPN license in the State of North Carolina
- CPR (BLS) Certification
- Ability to work effectively within healthcare systems and electronic medical records and follow our Service Standards.

Silas Creek Rehabilitation Center, located at 3350 Silas Creek Parkway, Winston Salem, NC 27103, is a 90-bed center that offers in-patient residential care, short-term rehabilitation, and specialty services. Our property features private and semi-private rooms, landscaped grounds, 7-day-per-week admissions, residential activities, and a rehabilitation gym with inpatient and outpatient therapy, and we accept a wide variety of insurance plans.
