MDS Nurse

The MDS Coordinator holds a critical position in ensuring our resident assessments are both accurate and optimized for individual care plans. Key responsibilities for this role encompass, but are not limited to:
Conducting thorough resident assessments (MDS) in accordance with all current regulations and guidelines
Ensuring the MDS process is completed accurately and promptly
Working closely with our interdisciplinary team to establish personalized care plans for residents
Monitoring resident progress and documenting responses to the implemented care plans
Engaging in the Quality Assurance and Performance Improvement (QAPI) initiatives
Facilitating effective communication between residents, their families, and other healthcare professionals to ensure superior care delivery
Remaining knowledgeable about the latest MDS regulations and guidelines for compliance
Part time/PRN
Pay rate: Up to $40 per hour.

Qualifications:
Must hold a current RN (Registered Nurse) license
RAC-CT (Resident Assessment Coordinator-Certified) certification is mandatory
In-depth knowledge of MDS 3.0, RAI process, and care planning methodologies
Exceptional organizational and communication skills, with strong interpersonal abilities
Capable of both independent and collaborative work within a team environment
Demonstrated commitment to delivering high-quality care to our residents
