Unified Residential Care is an Assisted Living Facility that specializes in providing comprehensive care and support to residents with Developmental Disabilities Administration (DDA) needs and Mental Health conditions. *Position Summary:*
The Licensed Practical Nurse (LPN) plays a key role in providing nursing care to residents in collaboration with Registered Nurses (RNs) and other healthcare professionals. The LPN will monitor resident health, administer medications, assist in daily activities, and ensure compliance with individualized care plans. This position requires a strong commitment to patient-centered care, particularly for residents with developmental disabilities and mental health needs.
*Key Responsibilities:*
* Provide direct nursing care to residents in accordance with their individualized care plans.
* Administer prescribed medications and monitor for side effects or adverse reactions.
* Assist with daily living activities such as feeding, bathing, dressing, and mobility as needed.
* Monitor and record vital signs, report any changes in resident health to the RN or physician, and update medical records accordingly.
* Collaborate with interdisciplinary teams to assess resident needs and develop comprehensive care plans.
* Provide emotional support and advocacy for residents, particularly in managing their mental health conditions.
* Educate residents and their families about health conditions, treatment plans, and medication management.
* Ensure compliance with state and federal healthcare regulations and the facility's policies and procedures.
* Participate in ongoing staff training, meetings, and quality improvement initiatives.
* Assist with maintaining a clean, safe, and therapeutic environment for residents.
*Qualifications:*
* Valid and active Licensed Practical Nurse (LPN) license in the state.
* Prior experience working in assisted living, long-term care, or with individuals who have developmental disabilities and/or mental health needs is highly preferred.
* Strong knowledge of medication administration and healthcare regulations.
* Excellent communication and interpersonal skills.
* Ability to work collaboratively in a team-oriented environment.
* Compassionate, patient-centered approach to care.
* Ability to handle physical demands such as lifting, standing, and moving residents as necessary.
Job Types: Full-time, Part-time
Pay: $34.00 per hour
