Social Worker

The *Social Worker (LMSW/LCSW)* is responsible to provide social work services to meet the consumer’s needs.

_ *Monday - Friday 8a-4p*_

*Duties and Responsibilities*
* Assesses consumers and completes appropriate sections of the Comprehensive Functional Assessment for consumers annually.
* Completes Initial Psychosocial Assessment and Comprehensive Psychosocial history as needed.
* Develops, writes, implements and reviews social work interventions as necessary to meet the consumer’s needs.
* Develops and provides in-service training for staff in regards to social work and social work interventions.
* Supervises social work assistants in regards to responsibilities by reviewing and signing off on work as required.
* Provides outreach to consumers, family members and/or other people with whom the consumer is involved.
* Coordinates with Accounting office efforts in regards to consumer financial resources.
* Participates as a member of the Interdisciplinary Treatment Team.
* Participates in consumer admission, movement and discharge procedures as needed.
* Participates in projects and committees as assigned.
* Attends and participates in Clinic and department meetings as necessary.
* Provides consultation for staff when requested in regards to suggestions, ideas and information as to more effective means of dealing with consumers.
* Interacts with consumers, clinical staff and other agency staff in a positive manner to facilitate the development of good working relationships and to enhance consumer relationships and their skill development.
* Liaison between the consumer/resident/DT and those community resources utilized by the consumers such as day programs, workshops, hospitals, etc.
* Ensures required information is maintained in both day habilitation and residential consumer records.
* Assists family members in obtaining legal guardianship when appropriate.
* Enforces and adheres to all safety policies and procedures.
* All other duties as assigned.

*Position Requirements and Qualifications*
* Master’s degree in Social Work
* Social Worker License
* One-year experience working or training in the field of Human Services
* Good oral and written communication skills
* Good interpersonal skills
* Valid NYS driver’s license
