Job Overview: We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN, you will play a crucial role in providing quality patient care and assisting in the coordination of healthcare services. Duties: - Administer medications and treatments as prescribed by physicians - Monitor and record patient vital signs, including temperature, pulse, and blood pressure - Assist with patient assessments and provide input for care planning - Collaborate with healthcare professionals to develop and implement patient care plans - Perform basic nursing procedures such as wound care, catheterization, and tracheostomy care - Provide health coaching and education to patients and their families - Assist with case management and coordination of healthcare services - Utilize electronic medical records (EMR) systems to document patient information accurately Skills: - Proficient in case management principles and practices - Knowledge of durable medical equipment (DME) usage and maintenance - Experience in emergency room (ER) settings is a plus - Familiarity with assisted living facilities and long-term care settings - Competent in IV infusion therapy techniques - Ability to provide person-centered care with empathy and compassion - Skilled in taking accurate vital signs measurements - Knowledge of ventilator management for patients requiring respiratory support - Strong health coaching abilities to support patients in achieving their wellness goals - Familiarity with ICD-10 coding system for accurate documentation If you are a dedicated LPN looking for an opportunity to make a difference in the lives of others, we encourage you to apply. Pay: $27.00 - $31.00 per hour
