Description:

PURPOSE:

To provide community counseling services to include outreach, intake, assessment, interventions and referral services for both individuals and groups.

KEY RESPONSIBILITIES:

Conduct intake assessment, psycho/social assessment, suicide risk assessment.
Open an average of six (6) new clients a month.
Provide crisis counseling and referral linkage to individuals and families.
Provide school based and/or “in-home" crisis counseling as needed.
Develop service plan in collaboration with clients and enter into NetMIS.
Address problems identified on the service plan with clients and families.
Responsible for client files and accurately recording sessions, meetings, phone calls and other pertinent information related to the client and family through progress notes and other documentation.
Provide group counseling activities and social skill instructions.
Participate in community outreach activities and document all outreach on appropriate form and submit back-up documentation for outreach to Operations for upload in NetMIS.
Participate in professional development activities, association meetings and trainings, as required.
Receives case and file review supervision from the Program Manager.
Participate in case staffing process and truancy court when needed.

Requirements:

LIST OTHER MINIMUM REQUIREMENTS:

Possess valid Florida Driver’s License in good standing and be insurable by the agency’s current insurance carrier. Must meet state criminal background check requirements.

EDUCATION AND EXPERIENCE:

Bachelor’s degree from an accredited university or college with a major in the field of counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field; and two (2) years of professional experience in the field of child welfare or social services.

Master’s degree from an accredited university or college with a major in the field of counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field; and one (1) year of professional experience in the field of child welfare or social services.
