*Key Responsibilities:*
As a RN Case Manager you will:
* Assess and evaluate the physical, psychosocial, and environmental needs of your assigned home hospice patients, using clinical records, on-site visits, and team consultations.
* Conduct initial and ongoing evaluations of each patient’s nursing needs during every visit.
* Communicate significant patient updates or changes in health to the appropriate clinical supervisor, physician, or team members involved in the patient’s care.
* Implement safety and care plans, offering guidance and instruction to patients and their families based on specific care needs.
* Ensure quality care is delivered by participating in the execution of individualized plans of care, focused on achieving positive patient outcomes.
* Recognize and respond to any changes in a patient’s condition, initiating appropriate actions when necessary.
* Confirm and follow the patient’s Plan of Care prior to every visit, providing care according to the physician's orders, clinical assessments, and best practices.
* Take on additional responsibilities and projects as assigned.
*What you'll Need to Bring:*
* Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
* Current State Licensee as a Registered Nurse RN
* 1 year of experience in a clinical care setting
* Experience in hospice or home health environment is preferred
* Nursing skills as defined by accepted nursing standards
* Ability to handle multiple priorities, documentation requirements, and deadlines
* Reliable transportation to perform job tasks
