The Visiting Nurse Association of Ohio is seeking a dedicated RN Case Manager to join our team in the East Cuyahoga territory. This full-time position involves providing comprehensive nursing care to patients across various age groups and health conditions.

Responsibilities:

Key Responsibilities:

Utilize the nursing process to assess, plan, implement, and evaluate patient care.
Provide preventive, therapeutic, and rehabilitative nursing services effectively.
Engage with patients and families through effective communication and interviewing techniques.
Collaborate with healthcare providers to ensure coordinated care.
Monitor patient comfort and safety, documenting significant changes and reporting them as needed.
Respond promptly to emergencies following established protocols.
Share relevant information with the healthcare team in a timely manner.
Qualifications:

Qualifications:

Experience: Minimum of 1-2 years in Med Surg or a hospital setting; home health care experience preferred.
Education: Completion of an accredited nursing program (diploma, Associate's, or Bachelor's degree; Bachelor's preferred).
Licensure: Registered Nurse licensed in the State of Ohio.
Skills: Strong organizational and planning abilities, attention to detail, and the ability to work under pressure.
Independence: Capable of working autonomously with minimal supervision while maintaining confidentiality.
Other: Valid driver's license and CPR certification required; bilingual skills are a plus.
