BAYADA Home Health Care is seeking an experienced Physical Therapist for a full-time, part-time or per diem opportunity performing home health visits in York, PA.
This office services adult and geriatric clients in Adams County.
A one year prior clinical experience as a licensed Physical Therapist is required.
As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients.
Responsibilities include:
Make home visits to clients in designated territories.
Perform diagnostic tests and measurements.
Develop and implement individualized care plans.
Continually assess and revise the care plan.
Educate and instruct clients and family members.
Accurately document client care management and services provided.
Qualifications include:
A current PA Physical Therapist license.
A minimum of one year of recent work experience as a Physical Therapist.
Graduation from a program approved by the American Physical Therapy Association.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem solving skills.
Solid computer skills.
