*This position delegates the responsibility and accountability necessary to carry out assigned duties in adherence with Company mission, values, policies and procedures.*
*Provides therapy services in accordance with physician’s orders and accepted professional practices.*
* *Registered therapists, develop an overall therapy plan of care to include: treatment and medical diagnosis warranting skilled intervention, planned duration and frequency to be provided, short and long term goals functional and measurable that support the discharge plan; Documents observations, patient’s response to skilled interventions/treatment programs, degree of progress and notable changes in patient’s condition within specific timeframes.*
* *Completes all documentation required timely and in accordance with state and federal regulations and policies/guidelines.*
* *Completes all billing required on the date of the provided skilled services, and in accordance with all state and federal regulations, policies/guidelines.*
* *Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.*
*Must hold Texas license/certification for appropriate discipline: PT.*
* *Ability to function effectively in multi-faceted systems and to interact effectively with all levels of staff and customers.*
* *Experience in a geriatric rehabilitation setting preferred.*
* *5,000 Sign on Bonus*
_*\*\*\**_*This Sign on Bonus is to be paid in 4 installments. The first payment of $1,000 will be made with the next payroll after the full-time employee as documented above has successfully completed 90 days of work without any disciplinary action. The second payment of $1,000 will be made with the payroll following the completion of 6 months of satisfactory work performance at full-time status. The third payment of $1,000 will be paid upon the successful completion of 9 months of full-time employment without any disciplinary action. The final payment of $2,000 will be paid upon the successful completion of one year of employment as a full-time employee, without any disciplinary action.*
