Quality Specialist RCFE / SNF Liaison The Center for Elders Independence is a PACE (Program of All-Inclusive Care for the Elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, including an A dult D ay H ealth C enter , and promoting autonomy, quality of life and the ability of individuals to live in their communities. The Position : Under the supervision of the Director of Quality Improvement, the Quality Specialist - RCFE/SNF Liaison is responsible for overseeing quality and regulatory compliance across contracted Residential Care Facilities for the Elderly (RCFEs) and Skilled Nursing Facilities (SNFs) . This role ensures CEI participants receive high-quality, person-centered care and that all care partners meet federal and state regulations. The Liaison also supports CEIs commitment to its Quality Improvement Program through annual reviews, collaboration on facility selection, and continuous performance monitoring. The Salary for the Quality Specialist RCFE/SNF Liaison role at Center for Elders Independence is $ 64,599 - $96,601 per year . The salary is based on the market for the Quality Specialist RCFE/SNF Liaison position, as well as experience, skills, abilities and work history Duties & Responsibilities : Conduct annual quality evaluations of all contracted RCFEs and SNFs . Perform initial quality reviews of potential facilities for contracting. Lead chart reviews for care plan compliance and communication between IDT and facilities. Serve as the primary liaison for quality-of-care concerns with contracted facilities. Ensure contracted facilities comply with federal and state regulations, including CMS and DHCS standards. Monitor external inspection reports and maintain directories of contracted facilities. Ensure all contracted facilities are in good standing and compliant with federal and state regulations . Actively participate in CEI workgroups to optimize RCFE/SNF processes and oversight tools . Facilitate alignment between external partners and CEIs PACE care delivery standards . Support IDT members by facilitating administrative communications with facilities. Support CEIs health information systems by contributing performance data and care outcomes . Document findings, interventions, and partner communications according to CEI protocols. Q ualifications : Bachelor s Degree in Nursing , Health Administration, Public Health, or related field. Minimum 2 years of experience in Residential Care Facilities and/or Skilled Nursing Facilities. Strong understanding of the PACE model and interdisciplinary team coordination. Familiarity with CMS, DHCS, and state licensing agency processes Familiarity with federal regulations governing PACE organizations and long-term care. Demonstrated experience with Microsoft Office Suite. Advanced proficiency with Word, Excel, and PowerPoint. Strong written and oral communication skills. Excellent customer service skills with demonstrated record of fostering positive business relationships. Ability to track, organize, and present information succinctly. Ability to exercise initiative and demonstrate flexibility. Exceptional planning and organizational skills.
