Skye Homecare is looking for an experienced LVN Clinical Coordinator.

Job Summary: Primary functions are to monitor and report day to day changes on clients of all ages in their place of residence, coordinate care while assuring processes are followed in QA department under the direct supervision of DON / Administration.

JOB QUALIFICATIONS-

Education: Graduate of an accredited Diploma, LVN licensed in Texas.

Licensure: Current Texas State license as an LVN, current Texas Drivers License.

Experience: 1-2 years of clerical experience, hospital, rehab, NH and HH preferred. Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR, negative TB screen, and Hepatitis profile.

Transportation: Reliable transportation is required. Valid and current auto liability insurance.

Environmental and Working Conditions: Works in office setting. May work in patient homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases. Ability to work a flexible schedule. Ability to travel locally.

Mental Effort: Ability to work under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition.

Essential functions: Monitor assigned cases to ensure compliance with requirements of Insurance Payor. Review telephone orders, QA Oasis. Teaching and training R/T, documentation requirements under supervision of the DON and/or ADON. Supervise and provide clinical direction to HHA to ensure quality and continuity of care.

Salary: $55,000.00 - $65,000.00 per year
