CNA/HHA preferred. 

The Direct Support Professional is responsible for safety and supervision of individuals with Intellectual Developmental Disabilities/Dual Diagnosis/Adult Autism

Duties:

• facilitate healthy and socially acceptable behaviors while living in community-based sites
• coordinate with program staff, implement residentially based interventions and assist the individual in achieving goals and outcomes as described in the ISP
• prompt or assist, as necessary, to ensure the individuals participate in the activities of daily living
• Engage the individual according to established supervision levels
• Assist in the process of identifying, developing and implementing the ISP
• Implement the goals from the individual's ISP
• Assist the individual with day to day activities and implement social skills.
• Support and Encourage individuals in movement towards greater independence.
• Support individuals to successfully integrate into the community, such as, teaching them transportation

skills, connecting them to community activities and resources, and helping them identify gainful

employment.
• Complete and submit all program documentation /data collection forms. • Communicate with staff at shift change about the status of the individual.
