SALARY RANGE: $16.36 - 18.00/ Hour


JOB DESCRIPTION:

  • Direct Support Professionals work with individuals or small groups of developmentally disabled individuals in the community.
  • They assist in developing the curriculum to teach functional life skills, independent living skills, social, direct care and pre-vocational skills to the individual.
  • Direct Support Professionals assist non-professional staff in the implementation of the individual’s treatment plans, serve as liaisons between agency and families, and carry out all tasks related to the individual.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Functions as a member of the interdisciplinary team in the formulation and the execution of the person’s Life Plan (LP).
  • Assist in the development of the individual’s Habilitation Plan, Safeguards and in meeting the objectives in their plans.
  • Assistance may be required in the following areas: cleaning, preparing snacks, social skills, budgeting, reading, math, computer skills, self-advocacy skills, volunteering, recreational and physical fitness activities.
  • Staff may need to provide hands on demonstration, modeling, prompting, and instruction in the use of these (and other) skills.
  • Completes all required Habilitation Services billing documentation in an accurate and timely manner.
  • Submits daily case notes that are complete, and accurate, reports and documents all incidents and maintain records.
  • Insures that consumer's chosen goals are implemented to improve or enhance the individual's independence, inclusion or integration into the community and that the outcomes are individualized.
