Nurse Practitioner Responsibilities

  • Conduct in-home wellness visits and risk adjustment health assessments, including:

    • Comprehensive assessments and physical examinations.

    • Medication history and prescription reviews.

    • Cognitive and mental health screenings.

    • Fall risk and social assessments.

    • Daily living activity evaluations and home environment reviews.

  • Provide health education to patients on topics like pain management and medications.

  • Build trust and strong relationships with patients through excellent communication and empathy.

  • Collaborate to close quality care gaps, such as screenings and labs.

  • Travel within your assigned area to provide care.

Nurse Practitioner Requirements

  • Valid, unencumbered Nurse Practitioner license in Oregon.

  • Travel within the assigned county (up to a 55-mile radius) and throughout the state as needed.

  • Previous in-home risk assessment experience preferred.

  • At least 3 years of patient care experience preferred (primary care/adult/geriatric).

  • Proficiency with EMR systems and ability to work independently.

  • Reliable transportation.

  • Bilingual skills are a plus.
