Job Description:

Meadowbrook Care Center is seeking a dedicated Physical Therapist to join our team on a full-time basis.

 

Responsibilities: -

·         Provide patient care and treatment using various physical therapy techniques

·         Assess and evaluate patients' conditions and develop individualized treatment plans.

·         Collaborate with other healthcare professionals to coordinate patient care. 

·         Monitor and document patients' progress and adjust treatment plans as necessary.

·         Educate instruct family members and caregivers of the patient on their treatment plan, and discharge planning

·         Maintain accurate and up-to-date patient records

Qualifications: 

·         Bachelor's or Master's degree in Physical Therapy from an accredited program.

·         Valid state licensure as a Physical Therapist.

·         Experience in acute and long-term care is preferred.

·         Strong knowledge of medical terminology, anatomy, and physiology.

·         Excellent communication and interpersonal skills.

·         Ability to work independently as well as part of a team.

·         Knowledge of NetHealth and Sigma care a plus

Additionally, our innovative therapeutic activities program includes manual arts, trips, entertainment, and focuses on the uniqueness and preferences of each individual.

We offer Outpatient Physical Rehab Services at flexible hours & strive to provide our residents with the utmost in top-quality care, provided by our experienced and dedicated staff.
