Job DescriptionJob Description

We are seeking a prn, part-time, or contract PT Physical Therapist and OT Occupational Therapists.  Needed to make home care visits and provide case management.

Responsibilities:

  • Develop and implement physical therapy programs that focus on rehabilitation
  • Contact physicians for orders and to coordinate care
  • Evaluate effects of therapy treatment and communicate patients' progress
  • Record and document patient care services using our online charting system
  • No OASIS if you wish the RN to complete instead

Qualifications:

  • Previous experience in therapy, rehabilitation, or other related field
  • Ability to handle physical workload
  • Ability to build rapport with patients
  • Excellent written and verbal communication skills
  • Strong leadership qualities
