Maintains confidentiality of all information regarding clients, staff, physicians, and GCLC obtained as a result of employment. Primarily responsible for appropriate documentation of service delivery, billing for services, and teaching of independent living skills as developed by the Psychosocial Rehabilitation Team in compliance with contractual, Medicaid, Medicare, Department of Mental Health and GCLC requirements. Documentation of services shall be in the form of progress notes including the beginning and ending time, the duration of billable service, the client’s daily goal and description of the participation that day. The implementation and service delivery will include but not be limited to, personal budgeting and financial management, basic living skills, coping skills, personal hygiene, comparative shopping, stress management, etc. Responsible for providing transportation services as needed for clients. Requisite, supervise and facilitate recreational and social activities. Responsible for ensuring the safety of all clients and leading meaningful activities within the program milieu. These activities may include but are not limited to: therapeutic socialization, classes, community meeting, and other such events that together constitute the schedule of the integrated program model. Must be able to demonstrate the ability to assist clients with disabling mental illness to achieve their maximum level of community integration and self-reliance. Assist client to learn to compensate for the effects of the symptoms of the illness through the development of new skills and a supportive environment and to counteract the effects of secondary symptoms by restoring a sense of confidence. Responsible for all other duties as assigned.
LEVEL AND DEGREE OF SUPERVISION:
Supervised by the PSI Team Leader with guidance from the Coordinator of Aftercare Services (CAS). Supervision to include all in-service training sessions, client staffing/team meetings, and general staff meetings and specialized training as required by Department of Health, Medicaid, PSI Team Leader, Aftercare Coordinator, etc.
QUALIFICATIONS:
Bachelor’s Degree in a human service area is required. Experience working with adults with Severely Disabling Mental Illness (SDMI) is preferred. Must have the ability to assess and utilize community resources. The personal characteristics and temperament suited to working with, caring for, and winning the respect of clients. Racial and ethnic backgrounds, which aid in creating a normal, responsive environment for the client in care. Must obtain CPR and Basic Life Saving Techniques offered by the Red Cross within the first year of employment. Must pass CYFD fingerprint clearance check and clearance with the Department of Motor Vehicles to maintain employment. Must complete a defensive driving course sponsored by GCLC within the first ninety (90) days of employment. Employee is responsible for obtaining continuing education to maintain appropriate licensure and to attend in-service training sessions designed to provide training in accordance with GCLC, DMH, and Medicaid standards. Preference given to qualified bilingual (Spanish/English) candidate.
