Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our Home Healthcare team, serving King County area. The ideal candidate will be responsible for the assessment and evaluation of patient care needs related to pain and other medical conditions. The PT will plan and administer personalized in-home therapy to help patients improve mobility, strength, and overall physical function, supporting them in achieving their rehabilitation goals.
Key Responsibilities
- Conduct thorough evaluations of patients to determine their physical therapy needs.
- Provide in-home physical therapy assessments and develop individualized treatment plans.
- Help patients restore function, improve mobility, and manage pain through therapeutic exercises and interventions.
- Work collaboratively with the home health team to ensure comprehensive care.
- Educate patients, family, and caregivers on rehabilitation processes, exercises, and home care strategies.
- Maintain accurate documentation of patient progress, therapy sessions, and treatment plans.
- Participate in interdisciplinary meetings to discuss patient outcomes and adjust care plans as needed.
Qualifications
- Degree from an accredited Physical Therapy Program.
- Current and unrestricted Physical Therapy license in Washington State.
- Minimum of one year of physical therapy experience preferred.
- Current CPR certification.
- Valid driver’s license, auto insurance, and reliable transportation for home visits.
- Ability to work independently and manage a flexible patient caseload.
- Excellent communication, time-management, and organizational skills.
- Ability to work collaboratively within a multidisciplinary team environment.
