Linn Community Nursing Home in Linn, KS is looking for one social service designee to join our 82 person strong team. We are located on 612 3rd St. Our ideal candidate is attentive, motivated, and engaged.

Responsibilities

  • Show prospective residents around the facility
  • Track census and the rooms available
  • Visit residents daily to observe their needs and concerns
  • Work closely with families regarding their loved ones
  • Conduct monthly resident council meetings and arrange necessary meetings with the families and the facility
  • New admission paperwork
  • Care plans for the residents and families with the facility
  • Assist with residents finances when necessary
  • Assist with applications for services needed
  • Assist with the activities department when needed
  • Handle roommate concerns should they arise
  • Advocate for the residents cares, needs and necessities
  • Follow HIPPA and security policies
  • Part of the leadership team
  • Potential to be called in regarding necessity or emergency

Qualifications

  • High school diploma
  • At least 18 years of age
  • Valid Driver License
  • Experience in geriatric cares
  • Basic computer skills
  • Bachelors Degree in Human Services; Psychology or Social Work Preferred
  • SSD License preferred
  • Long Term Care/ Subacute Experience Preferred

Hours

  • Typically Monday through Friday
  • Weekends and holidays may be necessary for this position
  • 40 hours per week
