POSITION SUMMARY:

PHYSICAL THERAPIST ASSISTANT/ PHYSICAL THERAPIST.

Services of the Physical Therapist Assistant (PTA) will be furnished under the supervision of a physical therapist. The PTA performs services planned, assigned, delegated and supervised by the physical therapist and functions as an active member of the home health team, providing services in accordance with the physician's orders and shall not exceed any of the duties authorized by the license issued by the State for the practice of physical therapist assistant. Lines of authority and reporting responsibilities: Report to the Director of Nursing, Clinical Manager, PT Supervisor, Administrator.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  1. Is licensed by the State Physical Therapist Licensing Board to practice as a Physical Therapy Assistant.Recommend a minimum of one year experience in an acute care facility.
  2. GENERAL REQUIREMENTS:

1. Must have genuine interest and concern for all clients with a special desire to work with the elderly client.

Participate in in-services program.

  1. Must be sufficiently mature and emotionally stable to work well with others.
  2. Must possess good physical and mental health.
  3. Must have a clean and neat appearance.
  4. Must be courteous and kind in manner.
  5. Can work harmoniously and skillfully with other staff members, patients and their families.
  6. Must be flexible and able to tolerate change or changing demands.
  7. Ability to promote the practice of physical therapy to the physicians, community, patients and their families. This includes the ability to verbally articulate and define physical therapy terminology.

 

OCCUPATIONAL THERAPIST.

This Agency hereby creates a position for the Occupational Therapist, indicated for functional limitations of activities of daily living that relates to the primary or secondary diagnosis, and shall be within the scope of practice authorized by the license issued by the State for the practice of occupational therapist. Such Occupational Therapist shall be governed by the following rules and responsibilities. Lines of authority and reporting responsibilities: Report to the Director of Nursing, Clinical Manager, OT Supervisor, Administrator.

He/she must be a graduate of an approved school for Occupational Therapist.

He/she must have a current state license for Occupational Therapist.

He/she must have a one year of experience in Occupational Therapy.

RESPONSIBILITIES

He/she shall provide Occupational Therapy services as prescribed by a Physician, physician assistant, or advanced registered nurse practitioner, acting within their scope of practice, which can be safely provided in the home. Conducts regular or OASIS assessments accurately, according to instructions in the OASIS Implementation Manual, and corresponding to documentation contained elsewhere in the assessment note. Participate in in-services program, and discharge planning.

He/she shall assist the physician, physician assistant, or advanced registered nurse practitioner, acting within their scope of practice, in evaluating the patient's level of function by applying diagnostic and therapeutic procedures.

He/she shall guide the patient in the patient's use of therapeutic creative and self-care activities for the purpose of improving function. Makes supervisory visits to all assigned Home Health Aides and OTAs.

He/she shall observe and record activities and findings in the clinical record and shall report to the physician, physician assistant, or advanced registered nurse practitioner patient's reaction to treatment and any changes in the patient's condition.

He/she shall instruct the patient, patient's family members and other health team personnel in certain phases of occupational Therapy.

Participate in QAPI program as needed.

Ensure HIPAA guidelines and procedures are maintained.

PTA / OTA Visit:          $55.00

HMO
