PHYSICAL THERAPIST-PARTTIME (Washington Pa}


Bridges Therapy Associates is seeking a committed Home Health Physical Therapist for a part­ time
position which will cover Washington, PA and surrounding areas.
The qualified candidate will provide effective, patient-centered services that will promote
functional improvement and maximize patient potential in their home environment. The success
candidate to join our team will have a professional work ethic and a desire to make a positive
difference in the lives of their patients.


QUALIFICATIONS:

. Proven work experience as a Physical Therapist preferred
. Valid Physical Therapy license to practice in Pennsylvania
. Valid driver's license and auto insurance
. Valid CPR
. Ability to manage patients of varying demographics, backgrounds, and diagnoses
. Must be willing to travel to patients' home
. Professional behavior
. Excellent communication, time management and interpersonal skills


RESPONSIBILITIES:

. Perform routine home physical therapy visits including evaluations, starts of care, daily visits,
reassessments, and discharges
. Provide high quality patient care, evaluates functional needs and outcomes, and deliver effective
and patient-centered treatments to achieve desired outcomes
. Designs and implements an appropriate plan of care for patients based on a thorough Assessment of
their functional needs in their home environment
. Conducts Physical Therapy practice within defined standards of care
. Communicates effectively with interdisciplinary team and support staff to achieve positive
patient outcomes
. Work with and communicate effectively with PTA staff as appropriate
. Perform and document appropriate visits in timely manner
