Job Summary: Our well-established home health agency is currently seeking an exceptional Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as a Private Duty Nurse (PDN). The successful candidate will provide skilled nursing care to a medically complex patient in their home setting, focusing on tracheostomy care, gastrostomy tube (G-tube) management, ventilator support, and other essential private duty nursing responsibilities. This position requires a high level of expertise, compassion, and dedication to patient-centered care.
Key Responsibilities:
- Perform comprehensive nursing assessments, develop individualized care plans, and implement nursing interventions to meet the patient's unique needs.
- Provide expert tracheostomy care, including suctioning, stoma care, and tracheostomy tube changes as needed.
- Manage and maintain the patient's G-tube, including formula preparation, feeding, medication administration, and stoma care.
- Operate, troubleshoot, and maintain ventilator equipment, ensuring proper settings and patient response.
- Administer medications via various routes (oral, enteral, parenteral) and monitor for therapeutic effectiveness and potential side effects.
- Monitor and record the patient's vital signs, respiratory status, neurological function, and other relevant clinical data.
- Collaborate with the patient's multidisciplinary healthcare team, including physicians, specialists, therapists, and social workers, to ensure seamless care coordination.
- Educate and train family members and caregivers on proper care techniques, equipment use, and emergency response protocols.
- Maintain meticulous documentation of all nursing interventions, assessments, and patient outcomes in accordance with agency policies and regulatory requirements.
- Participate in quality improvement initiatives, case conferences, and professional development opportunities to enhance patient care and nursing practice.
- Current, unencumbered LPN or RN license in the state of practice.
- Minimum of 3 years of clinical experience, with at least 2 years in a home health, private duty, or critical care setting.
- Demonstrated expertise in tracheostomy care, G-tube management, ventilator support, and other complex nursing interventions.
- Strong critical thinking, problem-solving, and decision-making skills.
- Excellent verbal and written communication skills, with the ability to effectively interact with patients, families, and healthcare professionals.
- Proficient in using electronic medical records and documentation systems.
- Current certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
- Ability to work independently, prioritize tasks, and manage multiple responsibilities in a dynamic home care environment.
- Flexibility to work various shifts, including nights, weekends, and holidays, to ensure continuity of care.
