Job Summary:
We are seeking a compassionate and dedicated Physical Therapist (PT) to join our skilled nursing facility (SNF) team. The PT will be responsible for evaluating, developing, and implementing individualized treatment plans for patients recovering from injury, surgery, or living with chronic conditions. The ideal candidate will help patients improve mobility, manage pain, and regain independence through therapeutic interventions and exercises tailored to their specific needs.

Key Responsibilities:

  • Patient Assessment: Perform initial evaluations of patients' physical abilities and limitations, review medical histories, and determine appropriate therapy goals.
  • Treatment Planning: Develop individualized treatment plans based on evaluation findings, patient goals, and physician recommendations. Modify plans as needed based on patient progress.
  • Therapeutic Interventions: Implement evidence-based physical therapy treatments, including exercises, manual therapy, and the use of specialized equipment to improve strength, balance, mobility, and overall function.
  • Patient Education: Educate patients and their families on home exercise programs, injury prevention, and ways to improve mobility and functionality outside of therapy sessions.
  • Progress Monitoring: Document patient progress and adjust treatment plans accordingly. Regularly report patient status to physicians and the interdisciplinary team.
  • Collaboration: Work closely with the interdisciplinary team, including occupational therapists, speech therapists, nursing staff, and physicians, to provide comprehensive patient care.
  • Compliance: Adhere to all state, federal, and facility regulations, including maintaining accurate and timely documentation for each patient.
  • Equipment and Safety: Ensure proper use of therapeutic equipment and maintain a safe environment for patients and staff.
  • Patient Advocacy: Serve as an advocate for patient needs, ensuring that the therapy provided aligns with their long-term goals for improved mobility and quality of life.

Qualifications:

  • Education: Doctorate or Master's degree in Physical Therapy from an accredited institution.
  • Licensure: Current state licensure as a Physical Therapist.
  • Experience: Previous experience in a SNF, long-term care, or geriatric setting is preferred but not required.
  • Skills:
    • Strong knowledge of physical therapy principles and practices.
    • Excellent communication and interpersonal skills.
    • Ability to work collaboratively within a multidisciplinary team.
    • Strong organizational and documentation skills.
    • Empathy and dedication to patient care.

Physical Requirements:

  • Ability to stand, walk, and move for extended periods.
  • Ability to lift and assist patients as needed.

Work Environment:

  • The position is based in a skilled nursing facility, and the PT will work with elderly and/or disabled patients who may have complex medical conditions.
