Job Overview:
We are seeking a skilled and compassionate Speech-Language Pathologist (SLP) to join our team at a skilled nursing facility (SNF). The SLP will work with residents to assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders. The ideal candidate will develop individualized care plans to help patients regain their independence and improve their quality of life. This role requires a strong understanding of geriatric care, an empathetic approach to patient needs, and the ability to work effectively within a multidisciplinary team.
Responsibilities:
Assessment and Diagnosis:
- Conduct comprehensive evaluations of residents' speech, language, cognitive-communication, and swallowing abilities.
- Interpret assessment findings to determine the level of impairment and set realistic treatment goals.
Treatment Planning and Implementation:
- Develop and implement individualized treatment plans based on residents' specific needs and goals.
- Provide direct therapy to address communication disorders, swallowing impairments, and cognitive deficits.
- Use evidence-based practices and interventions tailored to the geriatric population.
Collaboration and Communication:
- Collaborate with a multidisciplinary team, including physical therapists, occupational therapists, nurses, and physicians, to ensure coordinated and comprehensive care.
- Educate patients, family members, and staff on strategies to improve communication, safety, and quality of life.
Documentation and Compliance:
- Accurately document assessments, treatments, progress, and discharge summaries in compliance with SNF and regulatory guidelines.
- Maintain up-to-date records in electronic medical records (EMR) systems.
Patient and Family Education:
- Provide training and support to family members and caregivers on communication techniques and safe swallowing strategies.
- Educate patients on exercises and techniques to continue improving outside of therapy sessions.
Quality Improvement and Advocacy:
- Participate in facility initiatives to improve patient outcomes and enhance the quality of services provided.
- Advocate for patients' needs within the SNF and ensure ethical and compassionate care.
Qualifications:
- Master's degree in Speech-Language Pathology from an accredited institution.
- Current state licensure as a Speech-Language Pathologist (SLP).
- Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) preferred.
- Experience in a skilled nursing facility or with the geriatric population is preferred but not required.
- Strong communication, organizational, and interpersonal skills.
- Knowledge of Medicare/Medicaid regulations and documentation requirements for SNFs.
