Job Description:
We are seeking a compassionate and experienced Per Diem Nurse Practitioner to join our team. In this role, you will provide in-home primary and palliative care to geriatric patients, focusing on chronic disease management, symptom relief, and proactive care coordination. A key aspect of this position is discussing goals of care, completing advance directives, and ensuring that patients receive care aligned with their values and preferences.
Key Responsibilities:
Conduct comprehensive health assessments, diagnoses, and treatment plans for geriatric patients in their homes.
Manage chronic diseases, medications, and preventive care with a focus on reducing hospitalizations.
Provide palliative care and symptom management for patients with serious or complex conditions.
Lead goals-of-care discussions and assist patients and families in completing advance directives to ensure care aligns with their wishes.
Collaborate with caregivers, family members, and interdisciplinary teams to provide personalized, holistic care.
Document visits and care plans accurately and timely in the electronic health record (EHR).
Educate patients and families on disease processes, medication management, and advance care planning.
Qualifications:
Active Nurse Practitioner license in New York (CT a plus)
Minimum of 2 years of geriatric experience in primary care, home-based care, long-term care, or palliative care.
House call experience preferred but not required.
Strong communication and interpersonal skills, with a compassionate and patient-centered approach.
Ability to work independently, manage time effectively, and adapt to different home environments.
Reliable transportation and willingness to travel to patient homes.
Must have own Malpractice insurance
Schedule & Compensation:
1-2 days per week with potential for increased hours if a good fit.
Flexible scheduling.
Competitive per diem compensation.
