Type of Clinic: Outpatient Community Health Center / Federally Qualified Health Center (FQHC)
Position Status: Flexible Part-Time (or) Full-Time Schedules Available (4-10's or 5-8's)
Office Hours: Monday Friday | 7:00 a.m. 6:00 p.m.
Sign-on Bonus:$21,000 (Internal candidates that are currently employed here at NWHS are not eligible for a sign-on bonus.)
Referral Bonus: $5,000 (Referral bonus expire at the close of this job posting.)
Provider compensation at NWHS works on a salary basis.
The base salary for a Physician MD/DO, ranges from $212,888 to $266,163, and up to $319,384. Salary is DOE.

YOUR ROLE:

Make an impact today in the lives of people and become part of our primary care treatment team at our Total Health Community Clinic.
As part of the primary care treatment team, you will provide care coordination by working alongside our Community Health Workers, Medical Assistants, Nurses, Mental Health Case Managers, Counselors, Psychiatrists, Psych NPs, and other team members to support continuity of care for patients with complex needs. Our integrated care model means our medical and behavioral health clinicians together help patients and their families address medical conditions and behavioral health factors that affect their health and well-being.
YOUR TEAM:
Our multidisciplinary teams are fully integrated, allowing us to provide our patients with exceptional coordinated care.The leadership and Board of Directors at NWHS support our staff with a variety of training and development opportunities as we continue to expand our programs and community offerings. SPECIFIC DUTIES:
- Responsible for overseeing assigned patients care coordination including but not limited to specialty referral and follow up, case management, and enabling services. Provides professional medical services including examinations, diagnosis, and treatment planning for medical problems. Prepares and executes appropriate treatment plans and provides follow-up as needed while adhering to clinic policy. Consistently meets productivity expectations and procedures.
- Provides consultation when needed to nurse practitioners and physician assistants. Serves as supervising physician to no more than two mid-level providers. Proctors medical students as assigned.
- Maintains appropriate medical records documentation on all patients, in accordance with organizational policy. Consistently up to date on charting. Meets expectations of above 90% on weekly documentation report and maintains PAQ at less than 75 items at any given period.
- Actively contributes to care team by providing coverage to absent providers and being available to support staff and serving on improvement teams.
- Actively engaged in organizational initiatives such as 340b usage, Patient Centered Medical Home Certification, and Meaningful Use requirements.
- Works effectively in an integrated environment to coordinate care with dental and behavioral health professionals.
QUALIFICATIONS:
- MD/DO providers must have completed Primary Care residency (Family Practice, Internal Medicine, or Pediatrics) and be board certified.
- Licensed to practice in the State of Oregon with prescriptive authority and DEA
- Must be professionally bondable/insurable.
- Ability and willingness to provider Obstetrical care preferred.
- Team-oriented with strong interpersonal and communication skills.
- Valid Oregon Driver's License and proof of automobile insurance.
- CPR certification in infant, child and adult required.
- Bilingual/bicultural strongly preferred. Bilingual in English/Spanish a plus.
- Previous experience in community health preferred.
- Ability to function independently.
- Ability to work flexible hours and take call.
