2025-2026 School Nurse for The Constellations Program

Crescent City Schools is seeking a full-time (10-month) compassionate and experienced School Nurse to provide healthcare services to students with special needs, ensuring their health and safety while at school. This role involves collaborating with teachers, administrators, parents, and healthcare providers to create a supportive environment for students with chronic health conditions, disabilities, or medical needs that require attention during school hours. The Constellations Program's nurse will report to the Director of The Constellations Program.
Our School Nurses impact students' lives through:
Health Assessments and Care
- Conduct health assessments for students with special needs, including monitoring physical and emotional well-being.
- Provide care for students with chronic illnesses, disabilities, allergies, and other medical conditions requiring monitoring or intervention.
- Administer prescribed medications, treatments, and therapies according to students' Individualized Education Plans (IEPs) and Individualized Healthcare Plans (IHPs).
- Responsible for non-complex health procedures (enteral nutrition, toileting, hygiene, etc)
- Monitor and document health progress for students in the special education program.
Collaboration and Support
- Work closely with special education teachers, school counselors, and other staff to ensure that health accommodations are in place for students.
- Assist in developing and reviewing health-related aspects of students' IEPs, ensuring medical needs are adequately addressed
- Provide training and guidance for school staff on managing students' medical conditions and responding to emergencies.
Emergency Care and Response
- Respond promptly to medical emergencies or injuries, providing immediate care or first aid as needed.
- Maintain and update emergency medical information for students, ensuring accessibility in case of emergencies.
- Serve as a resource for staff in managing emergencies related to students, such as seizures, asthma attacks, or allergic reactions.
Health Education and Promotion
- Educate students and families about health-related topics, including nutrition, hygiene, and the importance of managing medical conditions.
- Support and educate parents or guardians on strategies to manage medical conditions or health interventions at school and home.
Health Records and Documentation
- Maintain accurate health records for all students in the special education program, ensuring compliance with privacy regulations (e.g., HIPAA).
- Track medical appointments, treatments, and any required school absences due to health issues.
Advocacy and Communication
- Advocate for students with special health needs, ensuring that necessary accommodations are in place for their success.
- Communicate regularly with families, caregivers, and healthcare providers regarding students' health needs and progress.
Professional Development
- Participate in professional development activities related to nursing, special education, and student health.
- Stay current with trends and best practices in school health services for students with special needs.

Physical Requirements
- Regularly required to sit, stand, walk, talk, hear, kneel, crouch, bend, operate a computer and other office equipment, reach with hands and arms, and must occasionally lift and/or move up to 80 pounds
- Must be physically able to perform Nonviolent Crisis Intervention training and techniques and react and intervene quickly during crisis situations, such as but not limited to the following circumstances: student elopement, physical altercations involving students, or any situation that requires physical intervention to maintain student safety
- This is an in-person role

About The Constellations Program
The Constellations Program is a preK through 4th grade school setting for students with low-incidence disabilities. At Constellations, we prepare students to achieve their highest independent level of functioning. The Constellations Program will support students in meeting their functional skill needs as well as the academic skills aligned to the LEAP Connect. While this program is geared primarily towards students with Autism, it is open to other students with limited to no verbal skills, intellectual impairments, and similar exceptionalities.
