PROGRAM: Youth Assertive Community Treatment HOURS PER WEEK: 35

REPORTING RELATIONSHIP: Youth ACT Team Leader

Salary Range: $57,000-$72,000

The Youth ACT Team serves children/youth with Serious Emotional Disturbance (SED) throughout Rockland County, who are returning home from inpatient settings or residential services, at risk of entering such settings, or have not adequately engaged or responded to treatment in more traditional community-based services. Youth ACT team delivers intensive, highly coordinated, individualized services and skilled therapeutic interventions through an integrated, multi-disciplinary team approach to better achieve success and maintain the child in the home, school and community.

SUMMARY of DUTIES

The Licensed Mental Health Professional is licensed by the New York State Education Department or is eligible for a Limited Permit and operates within the practitioner's scope of practice as defined in NYS law. These include, but are not limited to: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist. The Licensed Mental Health Professional is responsible for providing treatment to the child and their family/caregivers to address the clinical needs of the child and the complex needs of the family unit. Treatment interventions are to be individualized to the child/family and evidence-based practices should be used to address identified clinical and family system needs. The Licensed Mental Health Professional is an active member of the Youth ACT Team. Services are provided in the home and community and are delivered in a trauma informed, culturally and linguistically competent manner. The Licensed Mental Health Professional will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. When possible he/she will utilize evidence-based practices; always embracing a trauma-informed and person-centered approach.

SPECIFIC DUTIES and RESPONSIBILITIES:

  1. Conduct comprehensive, strength-based assessments of service and clinical needs.
  2. Develop a treatment plan in collaboration with the child, their family, and their care team.
  3. Communicate with school and other community providers regarding the child's progress.
  4. Provide crisis intervention as needed.
  5. Maintain up-to-date and complete case records, including documentation, and providing written reports and statistics as required.
  6. Participate in daily team meetings, consultations, and individual supervision.
  7. Due to the nature of the program, must have the ability to be flexible with working hours.
  8. Document progress and maintain a permanent record of activity according to established methods and procedures.
  9. Practice with a person-centered, trauma informed philosophy in alignment with the values of GMH.
  10. Follows the established agency policies and maintains appropriate professional standards.
  11. Provide 24-hour crisis intervention services on rotating basis.
  12. Adhere to all standards and guidelines established by applicable regulatory authorities.
  13. Other designated and related duties.

REQUIRED KNOWLEDGE AND SKILLS

  1. Principles, practices and methods of community mental health treatment including interviewing, assessment, family, child, and group counseling and crisis intervention.
  2. Adult and child psychology, including development and psychopathology.
  3. Applicable city, state and federal guidelines and laws affecting mental health treatment, services and reporting procedures.
  4. Characteristics and types of mental and emotional illness.
  5. Principles of psychological growth and development.
  6. Familiarity with and ability to use an Electronic Health Record system.
  7. Must complete the CANS-NY Training, be certified to use the CANS-NY, and maintain annual certification.
  8. Experience understanding family systems based on diversity of cultural and ethnic backgrounds.
  9. Highly organized, detail oriented, and self-motivated.

QUALIFICATIONS:

  1. The Licensed Mental Health Professional is licensed by the New York State Education Department and operate within the practitioner's scope of practice as defined in NYS law. These include, but are not limited to: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist. Individuals who are eligible for a Limited Permit in New York State will be considered.
  2. Basic Computer knowledge is required.
  3. Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
  4. Must have a valid Driver's License and a driving record that is satisfactory to our insurance carrier.
  5. Must maintain current New York State Defensive Driving certification.
  6. Must be cleared by the NYS Justice Center through fingerprinting for a Criminal History Records search.
  7. Must be cleared by the NYS Office of Children and Family Services for instances of child abuse and/or neglect.
  8. Must be legally eligible to work in the United States.
