Community Bridges is looking for full-time and part-time Therapeutic Direct Support Professionals for the START Center in Boscawen, NH

We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access and find creative solutions to develop meaningful community engagement.


The START Center is a six-bed, therapeutic program that is a short-term stay option for individuals with developmental disabilities and mental health concerns. At the Center, individuals work on therapeutic goals then return to their home community. The Therapeutic DSPs are trained to facilitate fun therapeutic groups, assist with daily living skills, and support individuals with coping skills. Strong communication skills are needed for this role (verbal and written). Ideal candidates will be calm, caring, and creative people who are engaging, take initiative, and are energetic. You must be comfortable with short-term interactions with individuals who may visit regularly or only on occasion.


Qualifications:

  • Education: Bachelor's degree and/ or associate's degree in a related field preferred. Years of experience can be substituted for college education.
  • Experience: Minimum of 2 years' experience supporting people with developmental disabilities required. (Reliable transportation, a valid driver's license, and proof of automobile insurance are required.)

Current Openings:

*Double shifts can be separated into to 6-hour long shifts or combined to increase hours to Full-Time/Part-Time capacity.

  • Tuesday & Friday (3pm - 11pm) - 16 hours
  • Friday (10am - 6pm) - 8 hours
  • Saturday (7am - 7pm) & (8am - 8 pm hour each shift) *
  • Sunday (8am - 8pm hours) *

Overnights

  • Monday - Thursday (11pm - 7am) - 32 hours
  • Friday (11pm - 8am) - 9 hours

Compensation and Benefits:

  • There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role. *Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.
  • Paid Time Off: For a full-time employee working 40 hours a week, hours of Paid Time Off is received per year PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO
  • Insurance: Medical, Dental, Vision, 403b with 1% company match, Excellent Paid Time Off, Life Insurance, Employer-paid Short Term Disability, and Employer paid Life Insurance.
  • Mileage Reimbursement: Employees are reimbursed for approved mileage driven on the clock.
  • Paid Training: As an employee, you are paid your usual rate of pay while attending training

Specific Duties and Responsibilities:

  • Provide direct care to guests of the START Center including assistance with personal care (bathing, personal and dental hygiene, and assistance with bathroom use) as necessary.
  • Provide structure and facilitate therapeutic group activities, including social skills building, communication, stress management, and daily living skills as outlined in the START Therapeutic Manual.
  • Provide de-escalation and intervention services for guests displaying emergency behavioral health concerns.
  • Assist guests to accomplish their therapeutic goals and objectives.
  • Assist with admission and discharge processes.
  • Communicate with guest's legal guardians and support system regarding their care at the
  • START Resource center and any significant events that occurred during their stay.
  • Communicate with the START Clinical Team regarding guests' responses to interventions while at the Resource Center and what is learned about behaviors/signs/symptoms, vulnerabilities, triggers, and effective interventions via daily notes, meeting attendance, or written summaries.
  • Perform all duties in a manner that demonstrates an awareness of and respect for each person's legal, civil, and human rights-including maintaining the confidentiality of information regarding the individual-and that ensures the respect and dignity of persons with an intellectual disability.
  • Complete documentation requirements as outlined such as data collection and daily progress notes. Meeting specific deadlines for documentation is an essential function of the role.
  • Complete incident reports, data collections, medication logs, and individual data accurately and in a timely manner (within regulation guidelines).
  • Assist in the orientation/training of new staff, which may include an introduction to an individual's support persons, behavioral strategies, START model orientation and an overview of START Resource center policies and procedures, and the START Resource
  • Center Behavior Plan.
  • Represent the agency in a professional manner at all times.
  • Attend discharge planning meetings, team meetings, agency and staff meetings, and other meetings, and/or conferences as requested by the supervisor.
  • Provide responsive, and clinically/medically based services and supports in accordance with guest needs, desires, and dreams. Such support will promote the dignity and respect of all individuals.
  • Coordinate the involvement of family members, outside professionals, and involved team members.
  • Promote and maintain satisfactory public relations between the residence, agency, and community.
  • Distribute medication in accordance with 1201 regulations.
  • Attend trainings and seminars as required. Meet and maintain all mandatory training requirements as a condition of employment.
  • Report safety, health, or human rights concerns immediately to the Supervisor.
  • Follow all applicable program policies and procedures of Community Bridges at all times while representing Community Bridges and the individuals we support.
  • Understand and ensure compliance with applicable state regulations.
  • Perform additional duties and responsibilities as assigned.

Work Environment and Physical Demands:

  • Work Environment- can be occasionally hectic with occasional periods of high stress. Must be able to maintain a positive and professional demeanor in a busy program environment.
  • Physical Demands- must be physically able to lift up to 50 lbs and to provide transfers as necessary. Walking and standing are required frequently with occasional stair climbing. Must be able to push wheelchairs and operate wheelchair lifts/Hoyer and barrier free lifts. Emergency behavioral intervention in accordance with MOAB techniques and individual behavioral plans may be required. Driving a passenger van and vehicle required frequently.

Travel:

Travel is required within the state frequently. Must have access to a personal vehicle during all scheduled work hours, maintain valid driver's license and appropriate insurance, and uphold and acceptable driving record. May be required to transport individuals and families receiving services on occasion.
