Position Overview
The Triage Registered Nurse (RN) is a pivotal healthcare advocate within the clinical setting. Collaborating closely with an integrated team of medical and behavioral health professionals, they focus on promoting and restoring patients' well-being by conducting comprehensive intake services, screening individuals for medical and psychiatric needs, and providing pharmacological treatment, historical trauma, substance use or dependence, and other conditions. Utilizing their nursing health expertise and advice, they guide patients on illness management and self-care, connecting them with essential resources and empowering them to make informed decisions about their health. With a trauma-informed care approach, they play a crucial role in sustaining patient flow through the clinic, managing the triage process, and scheduling visits.
Duties & Responsibilities
Triage Nurses with Fourth Street Clinic's role is to promote and restore patients' health by collaborating with physicians and multidisciplinary team members in the primary care setting, providing physical and psychological support to patients, and assisting with patients' flow through the clinic via the triage process.
Primary health care consists of a broad range of health care services, including the following:
- Promotion and maintenance of health care services, counseling, and patient education.
- Management of acutely and chronically ill patients and the provision of selected interventions according to established standards of health care.
- Consult and collaborate with primary care providers, other health care professionals, and community resources when appropriate.
The following information is intended to be representative of the essential functions performed by incumbents in this position and is not all-inclusive. The omission of a specific task or function will not preclude it from the position if the work is similar, related, or a logical extension of position responsibilities. This is a 40-hour, full-time position, which may include some fluctuation in hours worked per day. Triage Nurses will perform all nursing responsibilities when covering the clinic, and triage will complete additional responsibilities as assigned.
Principal Nursing Responsibilities
- Capable of working quickly and efficiently under pressure to triage all patients seeking same-day services. Work well with others to organize and efficiently manage the provider's clinic schedule based on triage medical and behavioral health acuity.
- Show sensitivity to the needs of unsheltered people while being innovative and assertive with a diverse population in various situations.
- Keeps customer service and the organization's mission in mind when interacting with all clients, co-workers, and others.
- Ensure the triage phone is answered during clinic hours with other nursing staff to coordinate with hospitals and community partners.
- Develop educational goals for individualized patient care, including learning objectives, a plan for educational content, and teaching methods with patients as needed.
- Evaluate the effectiveness of individualized follow-up plans for chronic disease management with patient and provider guidance.
- Utilize quality improvement initiatives with nursing staff to improve patient outcomes and achieve optimal health goals.
- Ensure all patient care actions are charted correctly and accurately in the electronic health record.
- Report all safety concerns with staff and patients to your supervisor immediately.
- General office skills, including phone etiquette, basic computer skills, and filing.
- Responsible for specific duties assigned to them by supervising personnel.
- Attend and participate in weekly staff, educational, and committee meetings as assigned.
- Comply with all clinic procedures for Univeral Data Set (UDS) measures and all standing orders.
- Maintain an active Registered Nurse license in the State of Utah.
- Work within the scope of licensure and seek consultation and guidance from providers as appropriate.
- Facilitate RN visits at all FSC sites.
- Expected to work at all FSC sites.
- All other clinic responsibilities.
Administrative
- Demonstrate interpersonal skills with all staff to support smooth office operations and teamwork to generate a positive attitude among FSC Staff.
- Utilize a proactive approach to managing assigned nursing orders, results, tasks, RN pool, and individual staff EPIC in baskets.
- Assist with the coordination of work for special projects, including but not limited to preventative health and the improvement of the Univeral Data Set (UDS) (mammogram, FIT testing, and pap clinic). The RN Supervisor must work with clinic staff on programs to improve patient outcomes. Assist the organization in quality improvement activities as assigned.
- Implementing new processes, administrative projects, and other duties as the Nursing Director assigns.
- Ensure that the clinic runs efficiently and addresses needs professionally and on time.
- Communicate departmental activities, changes in policies or procedures, and other related activities to appropriate personnel, committees, or others as determined.
- Ensures completion of incident reports as needed as outlined by policy.
Leadership Skills
- Demonstrates ability to communicate with all levels of clinic staff and providers.
- Able to execute daily job duties with an understanding of issues that need to be executed to a higher level of supervision.
- First-line problem-solving skills with the ability to listen, understand, and/or solve provider issues before they become more serious.
- A clear understanding of clinic policies and procedures to increase staff satisfaction and productivity levels.
- Ability to continually demonstrate the highest level of performance and behavior standards.
- Ability to be honest and straightforward, dealing with issues fairly and consistently.
- Ability to be a team player, support and assist team members, and be available to help and learn from the team. Keep an open mind to feedback and earn the trust of staff and providers.
- Excellent communication skills with staff, volunteers, patients, and community organizations involving FSC.
