The role of the RN-Case Manager supports our organization by ensuring patients are discharged from the hospital safely, identifying any barriers and working through those with the patient and care team.
Supervises front line staff in the day-to-day activities of the department.
Provides coaching and mentoring of staff. Interacts with staff daily, identify gaps/barriers to process, escalates and brings gaps/barriers to resolution. Assists and provides input to leaders with performance evaluations. Ensures staff compliance with departmental and organizational policies, procedures and standard work. Applies the existing body of evidence-based practice and scientific knowledge in health care to nursing practice, ensuring that nursing care is delivered based on patient’s age-specific needs and clinical needs as described in the department's Scope of Service. As an SSM Health nurse, I will demonstrate the professional nursing standards defined in the professional practice model. Uses the ANA Code of Ethics for Nurses to guide his/her response to the current and evolving health and nursing needs of our patients and our patient populations. Graduate of accredited school of nursing or education equivalency for licensing
Five years' acute care experience
Frequent keyboard use/data entry. Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
Registered Nurse (RN) - Missouri Division of Professional Registration
Registered Nurse (RN) - Oklahoma Board of Nursing (OBN)
