**Job Title:** Registered Nurse - Case Management

**Shift:** 8 Hour Days

**Guaranteed Hours:** 40 hours per week

**Experience:** Minimum of 1.5 years of general nursing, with at least 2 years required for first-time travelers

**License:** Registered Nurse - Board of Nursing

**Certifications:** Basic Life Support (BLS)

**Must-Have:** Experience in discharge planning and acute care case management, progression of care, and experience meeting with families to discuss plans of care

**Job Description:**

- Provide care for patients in all acute care departments
- Typically assigned to the same area during the contract, with minimal requirement to float to other areas
- Manage a caseload of 16-20 patients
- Work closely with case management assistants, lead case managers, social work complex psychosocial patient consultants, and utilization review nurses
- Utilize the EPIC charting system (experience required)
- Participate in a 1.5-week orientation including EPIC training
- Weekend rotation: 2 weekends per 13-week assignment
- Typical weekend staffing includes 3 RN case managers, 1 social worker, 1 lead case manager, 1 case manager assistant, and 1 utilization review nurse
- No on-call requirement
- Expected to cover 2 of the 4 major holidays
- Approval for up to 7 days off, with an extension required for any full week off; no time off allowed during early August
