Responsibilities of the DON Nurse:
Clinical Leadership
Oversee all clinical aspects of patient care, ensuring adherence to Medicare CoPs and state regulations.
Develop and implement policies and procedures for nursing services.
Ensure the delivery of high-quality, patient-centered care.
Responsible for reviewing OASIS-E, chart auditing, and ensuring complete, correct documentation that accurately reflects the Plan of Care.
OASIS-E Review (SOC/ROC/Recert/DC).
Supervising ICD-10 Coding, PCR and ADR preparation and submission.
Clinical Record Review (CRR).
Resource for resolving issues related to patient documentation.
Knowledge of various plan reimbursements (Medicare, Medicare Advantage, Medicaid, VA).
Review and evaluate all disciplines for quality, cost-efficient care plans.
Lead weekly care plan meetings and assist with quality assurance and improvement efforts.
Chart audit review and addressing areas of concern.
Knowledge of IDOA, IDPH, CHAPS, CMS Surveys/Audits is a plus.
Familiar with Financial Reimbursement Systems, PDGM, Case mix
Staff Management
Recruit, train, and supervise nursing staff and home health aides.
Conduct regular performance evaluations and promote professional development.
Schedule and coordinate staff assignments to meet patient needs.
Compliance and Quality Assurance
Ensure compliance with Medicare CoPs, including face-to-face encounter requirements.
Implement and maintain a quality assurance and performance improvement (QAPI) program.
Oversee the development and review of patient care plans every 60 days.
Administrative Duties
Collaborate with the leadership team on strategic planning and budgeting.
Maintain accurate clinical records and documentation.
Participate in on-call rotation to ensure 24/7 availability of nursing services.
Community Relations
Build and maintain relationships with healthcare providers and community partners.
Represent the agency at professional meetings and conferences.
Qualifications:
Minimum 5 years of home health nursing experience
At least 2 years of supervisory experience in home health
Active RN or LPN nurse license in the state of Illinois.
Proven expertise in QA for Home Health and OASIS-E review.
Proven experience with Axxess/Netsmart by MyUnity EMR Software.
Strong organizational skills with a solid understanding of home health care.
Knowledgeable about Medicare home health requirements.
Positive, professional demeanor, excellent team-building, communication, and interpersonal
Highly and consistently organized.
OASIS Quality Assurance skill.
Coding and ICD-10 knowledge.
Excellent communication and time-management skills.
