Position Overview:
The Case Manager plays a vital role in helping individuals or families navigate personal, health, and social challenges. By assessing needs, creating care plans, and monitoring progress, the Case Manager ensures claimants reach their highest level of functionality. You will work closely with family members, medical professionals, and social services to provide a comprehensive care approach.
Key Duties and Responsibilities:
Provide individualized support to claimants, tailoring services to meet their unique needs and goals.
Conduct comprehensive assessments using a person-centered approach, determining the care and rehabilitation needs of claimants.
Create and maintain case management plans, adapting them as needed based on the claimant’s progress and changing circumstances.
Advocate for claimants, establishing positive relationships with them and their families.
Provide crisis intervention and follow-up, making informed decisions regarding immediate risk factors.
Oversee the care regime, offering supervision and training to support staff.
Serve as a resource for claimants, providing information on equipment, housing, benefits, and other services.
Maintain accurate documentation and ensure compliance with program guidelines and HIPAA regulations.
Participate in clinical training to stay updated on best practices.
Perform other duties as assigned.
Qualifications:
A passion for supporting families
Ability to remain calm and effective in high-pressure situations.
Excellent communication skills, both written and verbal.
Proficiency with Microsoft Office Suite and relevant software applications.
Education & Experience:
Bachelor's degree in human services, healthcare, or a related field, or a relevant clinical certification.
3+ years of experience in a clinical environment, preferably working with children, persons with disabilities, or chronic illnesses.
Experience in care assessment, rehabilitation, and case management.
Previous supervisory experience is a plus.
Hours:
Monday through Friday, 8:00 AM to 5:00 PM with a 1-hour lunch, or 8:00 AM to 4:30 PM with a 30-minute lunch.
