Title: Manager - Case Management

Shift: Monday to Friday, 8:00 AM - 5:00/5:30 PM (on call every 5 or 6 weeks)

Location: 743 Spring Street, Gainesville, GA (Relocation assistance available: $3,000 - $8,000 depending on location)

Duration: Permanent

Salary: $114,800 - $166,000

Day-to-Day Responsibilities:

  • Manage the daily operations of the department, supporting the case management and interdisciplinary healthcare team in facilitating patient care through the continuum.
  • Monitor department activities to ensure optimal case management, including education, competency, and accountability of licensed staff.
  • Provide clinical oversight and training in collaboration with the Department Educator and leadership team to ensure staff competence.
  • Develop core case management standards and programs to facilitate optimal patient care and effective transitions to alternate levels of care.
  • Identify key performance indicators for the department, ensuring all professional and organizational standards are met.
  • Oversee recruitment, hiring, staff development, performance improvement, and progressive discipline as necessary.
  • Engage in program planning, budgeting, and long-term strategic planning for the department in alignment with organizational vision and goals.
  • Interact with patients across all age groups, adhering to population-specific guidelines and National Patient Safety Goals.
  • Provide cross coverage in all settings as required in the care continuum, including weekend rotation.

Must-Haves:

  • 4-5 years of case management experience in a large hospital (100 or more beds).
  • Leadership experience (team lead, manager, etc.).
  • Proven track record of initiating streamlined work processes (process improvements, etc.).
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint).
  • Empathetic/servant leadership style; culture is a significant focus for this team.
  • Bachelor’s Degree (does not have to be BSN; open to social workers if they meet other qualifications).
