Sunbelt Staffing is seeking a dedicated Nurse Practitioner to join an ISNP program in New York City, NY. This role focuses on providing patient-centered care in nursing homes and assisted living facilities to improve resident health outcomes.
Key Responsibilities:
- Conduct clinical assessments and develop care plans for residents.
- Provide on-site primary, preventative, and urgent care.
- Monitor high-risk patients and adjust care plans as needed.
- Collaborate with primary care physicians and facility staff.
- Maintain communication with residents families after each visit.
- Support smooth patient transitions from hospitals to the facility.
Qualifications:
- Active Nurse Practitioner license in New York.
