Job Summary:
- Role: RN Service Coordinator Level 1
- Objective: Enhance clinical and financial outcomes, member satisfaction, and manage care plans.
- Responsibilities: Manage member cases, supervise non-RN clinicians, develop and revise care plans, conduct assessments, and optimize member healthcare.
Essential Duties:
- Conduct holistic evaluations of members' needs.
- Provide education and support on care delivery models.
- Perform initial and follow-up assessments.
- Identify high-risk members and coordinate care.
- Manage members with chronic illnesses and disabilities.
- Develop and implement individualized care strategies.
- Collaborate with members, families, and healthcare providers.
- Initiate referrals for social services.
- Communicate health-related information to stakeholders.
- Advocate for patients and families.
- Document assessments and care plans.
- Participate in team meetings and utilization management rounds.
- Monitor service delivery and report complex cases.
Knowledge and Skills:
- Case management processes and person-centered care.
- Excellent communication and analytical skills.
- Leadership in clinical care teams.
- Knowledge of clinical resources.
- Data entry and word processing skills.
Required Qualifications:
- Current unrestricted RN license in Texas.
- Graduate of an accredited nursing school.
- 1+ years of experience with chronic illnesses, co-morbidities, and disabilities.
- Experience in community health settings.
- Proficiency in MS Word, Excel, and Outlook.
- Ability to travel within the assigned region.
- Reliable transportation and valid driver's license.
- Bilingual in Spanish.
