Position Summary:
Facility provides 24-hour per day residential care to extremely medically fragile pediatric (birth to five year-old) patients. We are licensed as a Pediatric Medical Group Home, providing a transitional care unit with planned lengths of stay between 6 and 24 months.
The Clinical Director has overall leadership and management responsibility for, including, but not limited to:
- Ensure the safe and consistent provision of literature-based medically intensive nursing care to at least eight to ten medically fragile pediatric patients aged birth to five years.
- Leadership and management, including direct supervision, evaluation, and mentorship of the pediatric critical care Registered Nurses (RNs) and Pediatric CNA's.
- Serve as the key point of contact in the ongoing coordination and communication with each patient’s care team, including the patient’s parents/legal guardians, primary care providers, specialty care providers, and the team
- Interface with cross-functional facilities team to ensure the facility is clean, professional. organized, adequately stocked with all necessary supplies
- Interface with cross-functional administrative teams to maintain governmental licensing and meet payor requirements
- Interface with hospital discharge coordinators to arrange new admissions; overall responsibility for optimizing patient census for each facility
- Normally on call 24/7; on-call responsibility may be delegated up to 50% of the time, covering at least four weekend days per month
- Identifying, coordinating and articulating measurable clinical outcome goals for each of medically fragile pediatric patients who typically present with the following:
- mechanical ventilation dependence (and progression towards weaning)
- tracheostomy management (and progression towards decannulation)
- literature-based progressive mobilization and optimization of each individual Patient’s strength, mobility and coordination
- advancing nutrition and hydration administration from current state at admission (i.e., TPN or continuous G- or J-TF) to G-TF bolus and/or safe po feeding
Duties/Responsibilities:
- Operation and management of pediatric critical field care facilities specifically designed for the intensive field care of medically fragile pediatric patients, birth to five years old.
- Work closely with the CEO supporting the marketing by:
- Developing and maintaining relationships with discharge coordinators, nurse case managers and other patient referral sources
- Review patient referrals and make recommendations on suitability for patient admission
- Managing patient admissions and discharges
- Support the mission to optimize patient outcomes and lead the evolution of the nursing profession
- Patient care goals established, measured and evaluated on a quarterly basis with the goal of safely discharging to the patient’s family’s home or when appropriate and indicated, to a more traditional pediatric medical group home
- Ensure all necessary patient care documentation is expeditiously made available for billing purposes
- Support continuous improvement efforts, both within the facility and companywide
- Direct supervision of RNs and PCTs:
- Conduct and document ongoing performance evaluation and feedback.
- Conduct and document performance evaluation and feedback including salary and pay recommendations; report to the President.
- Oversee scheduling of RN's and Pediatric CNA's.
- Review and approve team time entries for accuracy and completeness
- Ensure ongoing education, training and clinical mentorship is conducted and completed (as per Policy & Procedure) for all direct care RNs and PCTs
- Clinical Processes & Facility Oversight:
- Ensure the facilities are meticulously maintained and carefully cleaned for continuous safe operation
- Interface with and mentor RNs assigned to lead the individual patient’s care, update their provider plan of treatment and nursing plan of care
- Lead and coordinate training and continuing education of RNs in literature-based best nursing practice and clinical excellence. Mentor RNs who are applying to pursue advanced nursing education
- Oversee and mentor the Lead Pediatric CNA's and Lead RN's in training.
- Patient Access to Education:
- Oversee coordination of patients’ requisite medically intensive nursing care while the patients are accessing education
- Interface with school administration and educational personnel as necessary
- Review the patients’ Individual Education Plan with the patient’s RN Team Leader, patients’ parents, and educational providers
- Patient Discharge:
- Oversee, coordinate, evaluate and verify the patient’s parent mentorship process is conducted and documented prior to discharge
- Regular communication with each patient's team of primary and specialty care providers to coordinate continuous advancement to successful discharge
- Nursing care:
- Prescribed by the individual patient’s range of medical care teams and medical care providers
- Consistent with literature-based best nursing practic
- Optimize the health outcomes, functional outcomes and quality of life outcome goals established for the individual patient
- Respect the patient family’s goals and expectations, observing each patient’s cultural and religious traditions to the best of team capabilities
- Meticulously monitored and routinely updated based on the patient’s current physiologic state and progress towards health, function and quality of life goals
Required Skills/Abilities:
- Excellent leadership and management skills in a Pediatric Critical Care setting, leading, managing, mentoring, and inspiring Pediatric RNs and CNAs to bring the best of their knowledge, skills, and capabilities to apply literature-based best practice to:
- Identify and articulate realistic, measurable and time-linked patient goals, optimize each patient’s health outcomes, functional outcomes, and quality of life outcomes
- Excellent communication skills
- Outstanding ability and motivation to lead and work in a pediatric critical care team with other pediatric RNs
- Ability to accurately, reliably, and consistently meet the medically intensive nursing care needs of medically fragile pediatric patients in high stress situations
- Outstanding ability in meticulously evaluating and analyzing RN and PCT documentation for the purpose of tracking patient progress towards articulated goals and optimizing patient outcomes
- Able to work independently
- Comfortable interfacing with parents and a wide range of specialty care providers
- Excellent interpersonal, negotiation, and conflict resolution skills
- Excellent organizational skills and attention to detail
- Excellent time management skills with a proven ability to meet deadlines
- Strong analytical and problem-solving skills

Education and Experience:
Required Qualifications:
- Current RN license in Washington State
- MSN or MN required; doctoral preparation preferred
- Five to ten years of RN experience as an RN, with at least two years of clinical experience in one or more of the following:
- Critical Care Nursing
- Pediatric Nursing / Pediatric Critical Care
- Neonatal Nursing
- Nursing Management
- Neonatal and/or pediatric critical care and critical care
- Demonstrated ability to lead and manage teams of Pediatric RNs and PCTs
- Demonstrated ability to drive positive change in an innovative organization, whose goals include linking RN compensation with patient care outcomes
- Demonstrated ability to adapt to and safely adopt new technical systems and skills as needed.
- Demonstrated experience and clinical excellence in the provision of medically-intensive care, specifically with “infants” (birth to 12-months), “babies” (12 – 24 months), “toddlers” (2 to 5 years old), and/or special needs children.
Physical Requirements:
The role requires:
- the capability to work on-site at the facility for the duration of the 12-hour shifts as needed, or until relieved by the next scheduled RN
- ability / capability to make some repetitive motions, and
- the ability to lift infants, babies, and toddlers
- the ability to remain calm in emergency situations
- the ability to consistently and reliably ensure swift and accurate responses in critical care and in high stress situations
Note: Covering shifts is expected to be an exceedingly rare occurrence.
