Day-to-Day:

  • Working from their home (No requirement to come into office daily)
  • Once/Month meeting on site
  • Provided a company laptop and cell phone
  • Patient coverage currently divided into 4 territories (will be 5-6)
  • Will serve as Primary Care provider for 50-60 patients – range in frequency: once per 3mo – once per 2 weeks – Clinician will determine the appropriate care routine
  • Goal is 6 patients/day – Currently at 4-5 but will be bringing on more MAs to help with pre-appointment preparation and efficiency
  • MA will arrive earlier to do med rec, vital signs, etc. before provider conducts appointment
  • 3 different types of appointments: Primary Care (“longitudinal appointments”), Hospital + (patients discharged from the hospital - follow up visits: medications, movement, restroom usage, recommendations for home healthcare, intervention, etc.), Palliative Care (Home primary care)

Bonuses:

  • Bonus eligible for meeting productivity standards and coding accuracy
  • Compensated for $.67/miles
  • $5k/year reimbursement for CEUs, conferences, licensing, education
