We're looking for a skilled Clinical Pharmacist to join our healthcare team in Delaware In this role, you'll play a vital part in ensuring patients receive safe and effective medication therapies, working closely with doctors, nurses, and other healthcare professionals to enhance overall patient outes. This full-time position offers consistent Monday-to-Friday hours (9:30 AM to 6:00 PM), with no nights or weekends.

Shift Times: 9:30 AM - 6:00 PM, Monday - Friday
License Requirement: Must have Delaware Pharmacist License in hand
Position Type: Full-Time

Key Responsibilities:

  • Provide clinical pharmacy services to improve patient care and ensure medication safety.
  • Work closely with physicians, nurses, and other healthcare providers to manage medication therapies.
  • Review and monitor medication orders for appropriateness, dosage, and potential drug interactions.
  • Counsel patients on medication use and potential side effects.
  • Maintain accurate patient records and ensure pliance with regulatory requirements.
  • Assist with medication therapy management (MTM) and clinical protocols.
  • Monitor patient responses to medications and make remendations for adjustments as necessary.

Qualifications:

  • Doctor of Pharmacy (Pharm.D.) degree or Bachelor of Science in Pharmacy (B.S. Pharm.) with equivalent relevant experience.
  • Current, active Delaware Pharmacist License (must be in hand at the time of application).
  • Previous hospital pharmacy experience preferred, but retail pharmacists with clinical experience will be considered.
  • Strong munication and interpersonal skills, with the ability to collaborate effectively in a team environment.
  • Excellent attention to detail and mitment to patient safety.
