Nurse Practitioner

Sunbelt Staffing is seeking a dedicated Nurse Practitioner to join an ISNP program in Union, NJ. This role focuses on providing patient-centered care in nursing homes and assisted living facilities to improve resident health outcomes.

Key Responsibilities:

• Conduct clinical assessments and develop care plans for residents.
• Provide on-site primary, preventative, and urgent care.
• Monitor high-risk patients and adjust care plans as needed.
• Collaborate with primary care physicians and facility staff.
• Maintain communication with residents families after each visit.
• Support smooth patient transitions from hospitals to the facility.

Qualifications:

• Active Nurse Practitioner license in New Jersey.
• Experience in geriatrics or long-term care preferred.
• Strong communication and teamwork skills.
