Admissions Coordinator

We are seeking a dedicated and detail-oriented Admissions Coordinator to join our team. The ideal candidate will coordinate admissions for homecare, hospice, and skilled nursing facilities, ensuring that all pre-admission documentation is obtained and reviewed thoroughly. This role involves working closely with referring hospitals, physicians, skilled nursing facilities, patients, and families to verify physician orders and maintain positive relationships.
Responsibilities- Coordinate admissions for homecare, hospice, and skilled nursing facilities.
- Obtain pertinent pre-admission documentation from referral sources to determine if the agency can adequately care for the patient.
- Review clinical documentation to determine the level of care needed.
- Verify physician orders.
- Develop and maintain relationships internally and with community partners.
- Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families.

- Administrative support
- Administrative skills
- Minimum 1 year of healthcare experience
- Minimum 1 year of customer service or call center experience
- High school graduate or equivalent
- CNA/MA preferred, but not required
- Excellent customer service skills both in-person and over the phone
- Ability to work independently and as part of a team
- Basic computer knowledge
- Excellent organizational and analytical skills
- Ability to multitask in a fast-paced environment
- Good verbal and written communication skills
- Knowledge of Skilled Nursing, Homecare, and Hospice criteria

Work EnvironmentThe schedule for this position is Monday to Friday with a rotating weekly schedule of 8:30-5:00, 9:30-6:00, and 10:00-6:30. There will be one scheduled weekend every 4-5 weeks.
Job Type & LocationThis is a Contract to Hire position based out of Woods Cross, Utah.
