Job DescriptionJob DescriptionMinimum 1+ years pharmacy experience required.

We are seeking a licensed Illinois Pharmacist for a 13-week contract in an inpatient setting in Chicago.

Schedule: Monday Friday, rotating 1st and 2nd shift (7:30 AM 11:30 PM), with rotating weekends. (30 to 32 hours per week)

Responsibilities:

  • Provide comprehensive pharmaceutical care to patients receiving infusion therapy at home.
  • Review and interpret physician orders, ensuring accuracy and appropriateness of therapy.
  • Prepare and dispense sterile pharmaceuticals according to established protocols and regulatory requirements.
  • Monitor patient response to therapy and adjust treatment plans as necessary in collaboration with physicians and other healthcare providers.
  • Educate patients and caregivers on proper administration techniques, potential side effects, and medication management.
  • Conduct clinical assessments and communicate findings to the healthcare team.
  • Maintain accurate patient records and documentation of interventions.
  • Ensure compliance with all federal, state, and local regulations governing home infusion pharmacy practice.
  • Participate in quality improvement initiatives and continuous professional development activities.


Requirements:

  • Experience working in an inpatient pharmacy setting.
  • Cerner experience is a plus, but not required.
  • Clear and active Pharmacist license in the state of Illinois.
