LHH Recruitment Solutions is hiring a Medical Case Manager for our healthcare client located in the Oakland area. This position will be 100% onsite and a contract-to-hire opportunity, working a full-time Monday-Friday schedule. 

This case manager position will be working with complex care patients in order assist them along their ISP and help to get referrals and other care they need. This position will meet client's within the community, at hospital/clinic settings, and/or other locations in which the clients are located. Interested candidates must feel comfortable working with the homeless population, as this demographic will be a part of their caseload. 

Requirements 
  • High School Diploma or equivalent required, Bachelor's degree strongly desired
  • Minimum 2 years case management/social services experience 
  • Proficient within Microsoft Office and other software systems 
Skills 
  • Empathetic and compassionate towards others 
  • Excellent written and verbal communication skills
  • Knowledge of community resources for supportive services 
  • Tech savvy and can work within multiple software systems 
  • Ability to work well in a fast-paced environment and de-escalate situations as needed
  • Can make sound judgment calls in pressure situations and move forward in a productive manner 


Pay Details: $26.00 to $32.00 per hour
