We are seeking a skilled Geriatric Medicine physician to join our team at Archway Physician Recruitment LLC. As a Geriatric Medicine physician, you will play a vital role in providing high-quality care to seniors in our community.
**Key Responsibilities:**
- Provide comprehensive care to patients with acute and chronic illnesses, including assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
- Take full accountability for patient care and outcomes, seeking consultation from specialists when needed.
- Engage with hospitalists to ensure seamless care for patients in the hospital.
- Lead a care team consisting of a care promoter, care facilitator, and care coordinator to provide coordinated care for patients.
- Participate in the management of the center, covering for other providers as needed, and contributing to the recruitment of patients and additional providers.
**Requirements:**
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Experience in providing care to seniors with complex health needs.
- Strong communication and leadership skills.
