Job Summary:

CenterWell Home Health is seeking a skilled LPN (Licensed Practical Nurse) to join our team of compassionate healthcare professionals. As a Home Health LPN, you will provide personalized care to patients in the comfort of their own homes, focusing on their unique needs and goals.

Key Responsibilities:
  • Deliver high-quality nursing care under the direction of a RN, in accordance with state laws and CenterWell policies.
  • Develop and implement individualized care plans, incorporating patient preferences and goals.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless patient care.
  • Maintain accurate and detailed records of patient interactions, treatments, and outcomes.
  • Communicate effectively with patients, families, and caregivers to ensure informed decision-making and optimal outcomes.

Requirements:
  • Graduate of an accredited LPN program or equivalent experience.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • At least two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
