At Suncrest Hospice, we are committed to delivering exceptional care and service to our patients and their families. As a Certified Nursing Assistant/Home Health Aide, you will play a vital role in our team, working under the direction of a Case Manager/DON to provide resident care to assigned residents in accordance with resident assessments, care plans, and nursing management directives.
Key Responsibilities- Provide resident care to assigned residents, including assistance with daily living activities, personal care, and mobility support.
- Work collaboratively with other licensed care staff to coordinate resident care and ensure seamless delivery of services.
- Follow all Company nursing policies and procedures, as well as safety guidelines, to ensure a safe and healthy environment for residents.
- Participate in resident assessments, care planning, and goal setting to ensure individualized care plans are in place.
- Communicate effectively with residents, families, and healthcare team members to ensure comprehensive care and support.
- Certified Nursing Assistant (CNA) certification in the state(s) of practice.
- Home Health Aide (HHA) certification in the state(s) of practice.
- CPR certification.
- CNAWC certification.
- Strong commitment to improving quality of life while partnering with community organizations in patient care.
- Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and STNA leadership.
- Eager to gain higher-level opportunities such as certifications and further education.
- Able to work independently and on your own from a day-to-day basis.
- Must be willing to drive with reliable transportation, valid driver's license, and auto insurance.
- Competitive pay range of $20.00-$25.00 per hour.
